- Email Address
- Mobile phone number
- Mailing Address
- Certification Level
- Certification Expiration Date
- Primary Suffolk County Agency
- Additional agencies you are affiliated with
*NOTE – Your USERNAME cannot be changed! If you used your email address or name as your username, you must continue to use it to login.
